Fredag 8. november ble tilbudet FACT Ung Indre \u00d8stfold offisielt \u00e5pnet.<\/strong><\/p>\n\n\n\n

FACT Ung skal v\u00e6re et fleksibelt opps\u00f8kende behandlingsteam der barn og unge er en av de fire pasientgruppene som skal prioriteres i de nye helsefelleskapene. Dette krever at kommune- og spesialisthelsetjenesten samarbeider tett rundt utviklingen av et felles fundament. <\/p>\n\n\n\n

Grunnpilaren i FACT-modellen er at mennesker med sammensatte og alvorlige problemer skal ha mulighet til \u00e5 leve og delta i lokalsamfunnet, utenfor institusjonene. Med utgangspunkt i at det ordin\u00e6re hjelpeapparatet ikke klarer \u00e5 gi god nok hjelp til ungdommer med sammensatte og alvorlige tilstander, har FACT Ung blitt utviklet.<\/p>\n\n\n\n

M\u00e5lgruppen for FACT Ung<\/h3>\n\n\n\n

M\u00e5lgruppen er ungdom i alderen 12-18 \u00e5r som har alvorlig funksjonssvikt p\u00e5 flere omr\u00e5der i livet, sammensatte vansker og behov for langvarig og integrert innsats fra flere tjenester. Dette kan v\u00e6re psykiske vansker og\/eller rusvansker. Behandlingsfilosofien er \u00e5 ha fokus p\u00e5 ungdommens mestring, fungering og integrering i lokalsamfunnet. <\/p>\n\n\n\n

2. september 2024 startet teamet opp i Indre \u00d8stfold kommune og 2. oktober fikk de f\u00f8rste pasient. Da den offisielle \u00e5pningen ble holdt 8. november, hadde teamet tre pasienter.

Hvor finner du FACT Ung-teamet?<\/h3>\n\n\n\n

Teamet har sin base p\u00e5 helsestasjonen for ungdom (HFU), men de skal jobbe 80 % ute blant ungdom.<\/p>\n\n\n\n

\u00c5pningstidene er fra kl. 08.30-15.30 mandag til fredag. <\/p>\n\n\n\n

\u00c5pning av tilbudet i Indre \u00d8stfold kommune<\/h3>\n\n\n\n

Fredag 8. november var det offisiell \u00e5pning av FACT Ung i Indre \u00d8stfold kommune.<\/p>\n\n\n\n

Teamleder Avin Rostami \u00f8nsket velkommen og hun roste samarbeidet med sykehuset. – Det har v\u00e6rt et veldig godt samarbeid mellom Indre \u00d8stfold kommune og Sykehuset \u00d8stfold for \u00e5 f\u00e5 teamet p\u00e5 plass, sa Rostami.<\/p>\n\n\n\n

Varaordf\u00f8rer Kathrine Hest\u00f8 Hansen, avdelingssjef i BUPP Ragnhild Tran\u00f8y og Lill Margrete Eiken Hanssen som er ansvarlig for prosjektmidler holdt korte taler. De snakket om behovet for sammensatte langvarige ambulerende tjenester for \u00e5 hjelpe de ungdommene som har et sammensatt strev. Viktigheten av tverrfaglig tiln\u00e6rming og samarbeid mellom kommunen og spesialhelsetjenesten var ogs\u00e5 et viktig tema og her ble det gode samarbeidet mellom kommunen og sykehuset trukket fram. <\/p>\n\n\n\n
